We provide the meals and easy access – our solution automates the administrative process for both HCP recipients and Care Managers. This includes only charging 30% of the meal cost upfront and invoicing the Care Manager the remaining amount.

How HCP Meals Work.

  1. Sign up as a HCP customer via this link HERE or by asking your Care Provider to make contact with Lou at Table Made.

  2. Your application will be submitted automatically on completion and once your application has been approved by your Care Provider, you are ready to order! Place your order online HERE, whilst only paying 30% of cost of the meal at the time of ordering. We then invoice your Care Provider directly.

  3. Orders can be placed at any time by clicking on this link HERE or by calling Lou on 0425 221 199 on a Monday between 2 and 4pm to place your order over the phone. Phone orders are only accepted on this number on Mondays between 2-4pm.

  4. At the time of ordering you will choose a delivery time that suits your schedule. Deliveries are made Tuesday to Friday and dependent on your location. All meals are delivered frozen and in insulated recyclable packaging so if you are not home at the time of delivery they can be left in a safe place and will keep frozen for many hours.

FAQs

  • Home Care Packages (HCP) is a Government Program that enables eligible Senior Australians the ability to access funds to stay at home for longer. A key part of HCP is that these customers are only required to pay 30% of the cost of the meal upfront.

  • Eligibility is based on your care needs as determined through an assessment. You must also be an older person who needs coordinated services to help them stay at home. Find out more about assessment at My Aged Care.

  • If eligible, your Home Care Package could cover up to 70% of your food costs.

  • We use a platform called Cookaborough to connect with many different Care Providers. If we are not already connected to your Care Provider, Cookaborough will work to establish this connection for you.
